The reference fields are quote back fields for your use. These fields may make it easier for you to track or assign costs, for example. There are no rules about how you may use these fields. You may leave them all blank, choose pre-populated field labels for Fields 1 and 2, and/or you may use Reference Field 3 for a custom field. These fields and the values typed in when ordering backgrounds, will be quoted back to you on the final report.

- Choose "Employment-General Employment Screening" if you are an employer screening employees or prospective employees for general purposes. �
- Choose "Employment -Commercial Drivers-DOT/Safety" if you are an employer screening employees or prospective employees specifically for a Commercial Motor Vehicle driving position. �

This question asks if the fax number you are providing is for a fax situated so that it is protected from unauthorized prying eyes or public view. The information that we may fax to you is potentially very sensitive and could contain information that might be used by identity thieves. If the fax is not secure, for example sits on a counter in a high traffic area like a check out station or in a common area in an office shared by others not authorized to read personnel files, we will always call first before faxing to allow you time to station a trusted individual at that fax machine ready to pull off the sensitive fax. If a fax is secure and protected from public view, we will fax directly to that number when required to fax to you. Please do not check this box if the fax number you are supplying is for a fax machine which can be viewed or accessed by anyone not authorized to view your most sensitive personnel files.
